Several studies have supported the positive effect of respiratory rehabilitation (RR) in children and adolescents (CRA) with chronic respiratory diseases (CRD); however, qualitative aspects related to the experiences and perceptions about RR have been scarcely studied. OBJECTIVE: to analyze the qualitative evidence regarding the perceptions and experiences of patients, families and professionals related to the RR of children and adolescents with CKD. METHODS: Review of qualitative studies in 5 databases. We used MeSH terms and free English-language terms grouped into three dimensions: patients, intervention, and research design. The study subjects had to be patients, their families, teachers or treating health teams. No restrictions were placed on language or year of publication. The search strategy was configured as follows: ((Cystic fibrosis) OR (Asthma) OR (Neuromuscular diseases)) AND ((Respiratory rehabilitation) OR (Exercise)) AND ((Qualitative research) OR (Phenomenology) OR (Grounded theory) OR (Ethnography)). Two independent authors analyzed atingent titles, abstracts and long texts. Finally, a qualitative description of the results was made. RESULTS: Twenty-one qualitative studies were selected, all on patients, family members, teachers or professionals treating patients with cystic fibrosis (CF), asthma or neuromuscular diseases (NMD). Perception of benefits, parental influence, enjoyment of the protocols, and time required to engage in physical activity were categories identified in all three groups. Aspects such as perceived safety on school grounds and parental stress were specific categories, highlighted in the context of asthma and CF respectively. CONCLUSION: Several general and specific factors of a qualitative nature influence the experience of children with CRD during RR. Future studies conducted in our cultural context should be conducted to confirm these results.

OBJECTIVE: To describe the presence of obstructive sleep apnea syndrome (OSAS) in children with craniofacial anomalies (CFA), associate biodemographic characteristics and polygraph variables, and analyze the therapeutic management decided after the sleep study and the evaluation by a multidisciplinary team. PATIENTS AND METHOD: Retrospective study. Polygraphs were performed on patients aged between 1 month and 19 years with CFA. An initial and projected management was established categorized into ventilatory support, tracheostomy, surgery, dental, and medical treatment. Descrip tive and inferential statistics were performed, evaluating the association between demographic and polygraph variables and therapeutic management. RESULTS: 34 patients were included with a median age of 4.0 years (IQR 0.9 - 6.5). Diagnosis was 41.2% cleft lip and palate, 35.3% craniosynostosis, and 23.5% micrognathia. Polygraphs were altered in 70.6% of the cases; of these, 26.5% were diagnosed as mild, 5.9% moderate, and 38.2% severe OSAS. There was an association between minimum satu ration and diagnosis of OSAS (p = 0.0036), and in the presence of OSAS with the initial management applied (p=0.0013). There was no significant relationship between the different types of CFA with the initial therapeutic management (p = 0.6565). Initial and projected managements, respectively: Venti latory support (11.8% and 2.9%), tracheostomy (11.8% and 0%), surgery (35.2% and 26.5%), dental (20.6% and 53%), and medical treatment (20.6% and 17.6 %). CONCLUSIONS: 70% of the patients with CFA presented OSAS. The greatest severity was found in Cleft Lip and Palatine and Craniosynostosis. Therapeutic management was mainly oriented towards initial surgical and planned dental treatments based on the diagnosis of OSAS and not on the type of CFA.

INTRODUCTION: The submaximal tests are used to measure aerobic capacity and correlate with activities of daily living in chronic patients. The 6-min handbike cycle test (6mhct) was created for nonambulatory subjects, but there are no reference values to quantify and classify the cardiorespiratory fitness of children in this condition. OBJECTIVE(S): Generate 6mhct reference values and determine predictive variables in a population of Chilean children between 8 and 13 years old. MATERIALS AND METHODS: Cross-sectional study. Sampling for convenience, stratified by age. Evaluation of anthropometric variables, handgrip strength, heart rate (HR), effort perception, and blood pressure were performed, then the 6mhct was applied. Descriptive statistics, Student's t test and Mann-Whitney U test, correlation coefficient, and regression equation were used to estimate the total revolutions by age, being significant p < 0.05. RESULTS: Were evaluated 120 children homogeneously distributed by age. The total revolutions performed by boys and girls were 861.4 ± 102.9 and 771.8 ± 90.2, respectively (p < 0.001). There was a significant correlation between total revolutions and age (r = .52), weight (r = .29), height (r = .46), average  UULL (length of the upper limb) length (r = .44), average handgrip strength (r = .53), peak HR (r = .67), recovery HR (r = .44), and HR reserve (r = .72). The regression equation was established. CONCLUSIONS: These results can be used as preliminary reference values for the 6mhct in Chilean children from 8 to 13 years old. Handgrip strength, age, peak HR, and HR reserve influenced the performance of the 6mhct.

Frente a la pandemia del CoVID-19, resulta fundamental establecer estrategias manteniendo distanciamiento social, que permitan educar a profesionales de la salud sobre aspectos relacionados con SARS-CoV-2. La Sociedad Chilena de Neumología Pediátrica creó un E-learnig de contingencia de acceso libre. OBJETIVO: Describir las características del curso online "Desafíos de la pandemia CoVID-19 en Pediatría", y evaluar el grado de satisfacción de quienes lo cursaron. Estudio transversal. Todos los inscritos en el curso, entre mayo 2020 y julio 2020. Se grabaron 12 clases dictadas por especialistas con representación académica de Chile. Se realizó una prueba final y una encuesta de satisfacción, con puntaje de 1 a 7. Se realizó análisis descriptivo e inferencial, evaluando asociación entre clasificación final del curso, profesión y zona geográfica de los participantes con test de Kruskal Wallis, significancia estadística p<0,05. Aprobado por comité ética. Participaron 2541 alumnos, 73% mujeres. Mediana de edad 32,7 años. 90,7% de Chile, de estos 63,2% de regiones. De procedencia extranjera destacó Bolivia (2,9%) y Ecuador (2,5%). De los que realizaron la prueba, el 98,6% aprobó. Existió diferencia entre notas según profesión, Médicos-Kinesiólogos(p=0.025) y Médicos-Enfermeros(p=0.002); y según procedencia, Región Metropolitana (RM)-Otras regiones (p=0.041), RM-Internacional(p<0,001) y Otras regiones-Internacional(p<0,001). La evaluación de contenidos ponderó 6,6 y la accesibilidad 6,8. El 98% contestó que el curso cumplió sus expectativas. Concluimos que se inscribió un gran número de alumnos de Chile y Latinoamérica. Más del 98% aprobó la prueba final y refirió un alto nivel de satisfacción. Sugerimos emplear estrategias similares en situaciones de catástrofes sanitarias.


In the face of the CoVID-19 pandemic, it is essential to establish strategies, maintaining social distancing, that allow educating health professionals on aspects related to SARS-CoV-2. The Chilean Society of Pediatric Pulmonology created a free access contingency E-learning. OBJECTIVE: To describe the characteristics of the online course "Challenges of the CoVID-19 pandemic in Pediatrics", and to evaluate the grade of satisfaction of those who attended it.Transversal study. All those enrolled in the course, between May 2020 and July 2020. 12 classes taught by specialists with academic representation from Chile were recorded. A final test and a satisfaction survey were carried out, with a score from 1 to 7. A descriptive and inferential analysis was implemented, evaluating the association between the final classification of the course, profession and geographical area of the participants with the Kruskal Wallis test, statistical significance p< 0.05. Approved by ethics committee. 2541 students participated, 73% women. Median age 32.7 years. 90.7% from Chile, of these 63.2% from regions. Of foreign origin, Bolivia (2.9%) and Ecuador (2.5%) stand out. Of those who took the test, 98.6% were successful. There was a difference between grades according to profession, Physicians-Kinesiologists(p=0.025) and Physicians-Nurses(p=0.002); and according to origin, Metropolitan Region (RM)-Other regions (p=0.041), RM-International (p<0.001) and Other regions-International (p<0.001). The content evaluation pondered 6.6 and accessibility 6.8. 98% responded that the course met their expectations. CONCLUSION: A large number of students from Chile and Latin America were enrolled. More than 98% passed the final test and reported a high level of satisfaction. We suggest using similar strategies in situations of health catastrophes.

Introducción: Los pacientes hospitalizados con altas dependencias tecnológicas respiratorias son cada vez más frecuentes y generan largas estadías en unidades de cuidados intensivos. Las estrategias que mitiguen su impacto han sido escasamente descritas. Objetivo: Describir 6 años de experiencia de una Unidad de Ventilación Mecánica Prolongada Pediátrica.Métodos: Estudio retrospectivo. Se incluyeron todos los niños ingresados a la Unidad entre 10-2012 y 12-2018. Se realizó estadística descriptiva e inferencial, analizando tiempos de hospitalización y reingresos. Se compararon distintas variables según tipo de patología y ventilación mecánica.Resultados: 113 pacientes registraron 310 ingresos a la Unidad. Edad de ingreso: 2,2 años (0,6-8,8); varones: el 60,2 %. Patologías: enfermedad neuromuscular (el 22,1 %), enfermedad pulmonar crónica (el 20,4 %), daño neurológico (el 34,5 %), obstrucción de vía aérea superior (el 9,7 %), cardiopatía (el 3,5 %), síndrome de Down (el 9,7 %). Se utilizaron 10 507 días/cama; con índice ocupacional del 92,6 %, el 54,8 % de traslados a la Unidad de Cuidados Intensivos y el 66,1 % de reingresos. Hospitalización media: 16 días (6,5-49,0); diferencias en edad de ingreso según patologías (p = 0,032). Hubo más reingresos en niños con daño neurológico y síndrome de Down (p = 0,004). Los niños con asistencia ventilatoria invasiva presentaron más días de hospitalización (p < 0,001) y reingresos (p < 0,001).Conclusión: El índice ocupacional fue superior al 90 %; permitió mayor disponibilidad de camas intensivas y egresar a todos los pacientes. Los niños con asistencia ventilatoria invasiva se hospitalizaron más tiempo y reingresaron más

More readmissions were observed in children with neurological damage and Down syndrome (p = 0.004). Children with invasive ventilation were observed to have a longer length of stay (p < 0.001) and more readmissions (p < 0.001).Conclusion: The occupancy rate at the PMVU was over 90 %, which allowed more available intensive care beds and discharging all patients. Children with invasive ventilation had a longer length of stay and more readmissions.

ABSTRACT Objective: To evaluate the strength of respiratory muscles and to compare maximum inspiratory (MIP) and expiratory (MEP) pressure and MEP/MIP ratio between patients with chronic respiratory diseases and healthy individuals. Methods: Case-control study. Individuals with neuromuscular disease and post-infectious bronchiolitis obliterans were considered. In addition, they were also matched according to anthropometric and demographic characteristics with healthy children and adolescents. MIP, MEP in the three groups, and pulmonary function only in patients with chronic respiratory diseases were recorded. Results: A total of 52 subjects with CRD (25 with neuromuscular disease, and 27 with post-infectious bronchiolitis obliterans) and 85 healthy individuals were included, with an average age of 11.3±2.1 years. Patients with neuromuscular disease and post-infectious bronchiolitis obliterans presented lower MIP and MEP when compared with healthy individuals, although MEP/MIP ratio was lower in patients with neuromuscular disease (0.87±0.3) and higher in patients with post-infectious bronchiolitis obliterans (1.1±0.3) compared to the healthy group (0.97±0.2). Only in patients with neuromuscular disease a negative correlation was observed between MEP/MIP ratio and age (r=-0.50; p=0.01). Conclusions: Differences in the pattern of muscular weakness between patients with chronic respiratory diseases were observed. In patients with neuromuscular disease, a decrease in the MEP/MIP ratio depending on MIP was verified; and in those patients with post-infectious bronchiolitis obliterans, an increase in the MEP/MIP ratio depending on MIP was also observed.


RESUMO Objetivo: Avaliar a força dos músculos respiratórios e comparar a relação entre a pressão expiratória máxima (PEmáx) e a pressão inspiratória máxima (PImáx) em pacientes com doença respiratória crônica (DRC) e crianças saudáveis. Métodos: Estudo caso-controle. Foram selecionados indivíduos com doença neuromuscular e bronquiolite obliterante pós-infecciosa. Ademais, os grupos foram pareados com crianças e adolescentes saudáveis, considerando características antropométricas e demográficas. Foram registradas a PImáx e a PEmáx nos três grupos e a função pulmonar apenas em pacientes com doença respiratória crônica. Resultados: Foram incluídos 52 indivíduos com DRC (25 com doença neuromuscular e 27 com bronquiolite obliterante pós-infecciosa) e 85 indivíduos saudáveis, com idade média de 11,3±2,1 anos. Pacientes com doença neuromuscular e bronquiolite obliterante pós-infecciosa apresentaram menor PImáx e PEmáx em comparação aos indivíduos saudáveis, embora a relação PEmáx/PImáx tenha sido menor nos pacientes com doença neuromuscular (0,87±0,3) e maior nos pacientes com bronquiolite obliterante pós-infecciosa (1,1±0,3) em comparação ao grupo saudável (0,97±0,2). Somente em pacientes com doença neuromuscular foi observada uma correlação negativa entre a razão PEmáx/PImáx e a idade (r=-0,50; p=0,01). Conclusões: Foram observadas diferenças no padrão de fraqueza muscular em pacientes com doença respiratória crônica. Nos pacientes com doença neuromuscular, verificou-se diminuição na relação PEmáx/PImáx dependendo da PImáx; em pacientes com bronquiolite obliterante pós-infecciosa, foi observado aumento na relação dependendo da PImáx.

RESUMO

El diagnóstico de apneas en lactantes menores de tres meses constituye un gran desafío y es un área en pleno desarrollo. Es por esto, que diferentes especialistas en sueño, pertenecientes a dos Sociedades Científicas de Chile: la Comisión de Sueño, de la Sociedad Chilena de Neumología Pediátrica (SOCHINEP) y el Grupo de Trabajo Trastornos del Sueño en Pediatría de la Sociedad de Psiquiatría y Neurología de la Infancia y Adolescencia (SOPNIA), se han puesto de acuerdo en proponer un consenso básico sobre los parámetros de sueño y del manejo de las apneas en los menores de 3 meses de vida. El objetivo, es que podamos contribuir al manejo de estos pacientes, con un lenguaje y manejo similar, y valores de referencia apropiados para ese grupo etario, respaldado con las últimas investigaciones al respecto.


Sleep apnea diagnosis in infants younger than 3 months has been a major challenge for modern medicine. Using current literature, experts from the Chilean Society of Pediatric Pulmonology Sleep Commission, and the Chilean Society of Psychiatry and Neurology for Children and Adolescents, have produced a national state-of-the-art consensus. RESUMO

The CoVID-19 pandemic may limit health resources in the hospital setting for the management of seriously ill patients, mainly adults; For this reason, strategies should be established from the pediatric field, which can consequently have an impact on the number of beds usually assigned to the child population. Of the patients who use prolonged mechanical ventilation, a significant number are managed at home, being considered to be at high risk of hospitalization and complications; on the other hand, there are some of them that remain institutionalized for clinical and / or social reasons. This article aims to provide guidelines for its safe handling both at home and in the pediatric services of our country, during times of pandemic CoVID - 19.


La pandemia de CoVID-19, puede limitar el recurso en salud en el escenario intrahospitalario para el manejo de pacientes graves, principalmente adultos; por lo cual se deben establecer estrategias desde el ámbito pediátrico, que pueden repercutir consecuentemente en los días camas destinados habitualmente a la población infantil. De los pacientes que emplean ventilación mecánica prolongada, un número importante es manejado en domicilio, siendo considerados de alto riesgo de hospitalización y complicaciones; por otro lado, existen algunos de ellos que permanecen institucionalizados por razones clínicas y/o sociales. Este artículo pretende entregar orientaciones para su manejo seguro, tanto en domicilio como en los servicios de pediatría de nuestro país, durante tiempos de pandemia CoVID ­ 19.

INTRODUCTION: The EPInfant scale has been validated for the perceptual estimation of physical exer tion in Chilean children, but its usefulness for self-regulation of exercise intensity is unknown. The objective of the study was to evaluate the criterion validity of the EPInfant scale to regulate and re produce exercise intensity in a sample of healthy children. SUBJECTS AND METHOD: 15 children between the ages of eight and 12 were selected for an incremental exercise test (IET) and three perceptually regulated exercise tests (PRET) on a treadmill. The tests were performed with a 48-hour interval between them. In the PRET, the exercise load was adjusted perceptually for five minutes, randomly considering levels 3, 6, and 9 of the EPInfant scale. The average heart rate (HR) during PRET was considered as the perceptually reproduced intensity. Variance analysis, simple linear regression, and reliability analysis were used to determine the reproducibility of HR during PRET. RESULTS: There was a significant difference in HR between perceptual levels during PRET (p < 0.001). Additionally, a correlation was observed between HR during the IET and the PRET (r = 0.83, r2 = 0.69). The intra class correlation coefficient was 0.76, 0.83. and 0.93 at perceptual levels 3, 6, and 9; and the mean dis cordance between HR during the IET and the PRET was -2.4 beats/min. CONCLUSION: In the studied sample, the EPInfant scale was valid to allow the perceptual regulation and reproduction of exercise intensity in a treadmill.

Introducción. La rehabilitación respiratoria infantil es relevante en el manejo de enfermedades respiratorias crónicas. En Chile, se desarrolla solo en algunos hospitales.Objetivo. Describir los resultados de un programa de rehabilitación respiratoria infantil tras 3 meses de rehabilitación.Metodología. Estudio retrospectivo de los pacientes con enfermedad pulmonar crónica, enfermedad neuromuscular y otras patologías crónicas admitidos desde junio de 2011 a junio de 2017. Se realizó evaluación física general, evaluación de musculatura respiratoria y espirometría. Se planificó entrenamiento físico general y entrenamiento muscular respiratorio, con protocolos institucional, mixto y domiciliario.Resultados. Ingresaron al programa 156 pacientes (11,45 ± 3,55 años), 68 con enfermedad pulmonar crónica (11,56 ± 4,6 años, el 43,5 %), 45 con enfermedad neuromuscular (11,4 ± 3,7 años, el 28,8 %) y 43 con patologías crónicas diversas (11,31 ± 3,9 años). Del total, se entrenaron 102 (el 65,4 %); dirigido (n = 27), mixto (n = 23) y domiciliario (n = 50) con adherencia del 69 %, del 87,5 % y del 70 %, respectivamente. Aumentó el 19,3 % la presión inspiratoria máxima (p = 0,001), el 14,5 % la presión espiratoria máxima (p = 0,001), el 12,9 % la capacidad vital forzada (p = 0,001), el 11,6 % el volumen espiratorio forzado al primer segundo (p = 0,004) y 17,6 metros en el test de marcha de 6 minutos (p = 0,036) tras 3 meses de rehabilitación.Conclusiones. El protocolo de intervención incrementó el estatus funcional cardiorrespiratorio. La adherencia fue superior al 65 %.

INTRODUCCIÓN: La escala EPInfant ha sido validada para la estimación perceptual del esfuerzo físico en niños chile nos, pero se desconoce su utilidad para la autorregulación de la intensidad del ejercicio. OBJETIVO: evaluar la validez de criterio de la escala EPInfant para regular y reproducir la intensidad de ejercicio en una muestra de niños sanos. SUJETOS Y MÉTODO: Se seleccionaron 15 niños entre 8 y 12 años, en quienes se realizó un test de carga incremental (TCI) y tres pruebas de regulación perceptual (PRP) en trotadora. Las pruebas se realizaron con un intervalo de 48 horas entre ellas. En la PRP, la carga de trabajo se ajustó perceptualmente durante 5 minutos, considerando aleatoriamente los niveles 3, 6 y 9 de la escala EPInfant. Se consideró el promedio de la FC durante la PRP como la intensidad reproducida perceptualmente. Se empleó análisis de varianza, regresión lineal simple y análisis de confiabilidad para determinar la reproducibilidad de la FC durante la PRP. RESULTADOS: Existió diferencia significativa en la FC entre los niveles perceptivos durante la PRP (p < 0,001). Adicional mente, se observó correlación entre la FC durante el TCI y la PRP (r = 0,83; r2 = 0,69). El coeficiente de correlación intraclase fue de 0,76, 0,83 y 0,93 en los niveles perceptivos 3, 6 y 9; y la discordancia media entre la FC durante el TCI y la PRP fue de -2,4 lat/min. CONCLUSIÓN: En la muestra estudiada, la escala EPInfant fue válida para permitir la regulación y reproducción perceptual de la intensidad del ejercicio en trotadora.

INTRODUCTION: The early diagnosis of Sleep Disordered Breathing (SDB) may allow proper intervention. Currently, polygraphy (PG) is a reliable and accessible alternative. OBJECTIVE: To describe and analyze the PG of children > 1 year old with suspicion of SDB. PATIENTS AND METHOD: PG of children > 1 year old and adolescents from Concepcion, Chile, with suspected SDB were included, from December 2011 to Au gust 2017. Demographic, clinical and polygraphic variables were collected. It was used descriptive sta tistics, expressing results in median and range. The association between apnea-hypopnea index (AHI) and oxygen saturation was determined by Spearman's Rho, considering significance of p < 0.05. Re sults: 190 studies were analyzed. Age 7.9 years old (1.0-20.6), 61% males. DIAGNOSIS: neuromuscular disease (NMD) (24.2%), chronic lung damage (21.1%), upper airway obstruction (UAO) (19.5%), neurological damage (11%), Down syndrome (8.9%), upper airway malformations (7.4%), central hypoventilation (3.7%), obesity (2.6%), and others (1.6%). 55.3% were altered PG, with 53.3% of mild Obstructive Sleep Apnea Hypopnea Syndrome (OSAHS), 30.5% moderate, and 16.2% severe. There were no significant differences in AHI between groups of pathologies (p = 0.032), highlighting a higher AHI in obese patients 9 (0.41-51), and those with NMD 23.9 (0.4-36.6). It was found asso ciation between AHI and oxygen saturation parameters: mean saturation (rho = -0.425; p = 0.001), minimum (rho = -0.654; p = 0.001), and oxygen saturation below 90% (rho = 0.323; p = 0.001) in the whole sample. DISCUSSION: There was a high percentage of OSAHS in at-risk pediatric patients, especially in those with NMD and obesity. PG is an accessible and implementable tool in a public hospital, a situation that can potentially be extrapolated to other healthcare centers.

INTRODUCCIÓN: El diagnóstico precoz de los Trastornos Respiratorios del Sueño (TRS) puede permitir una intervención oportuna. La poligrafía (PG) es una alternativa confiable y accesible en la actualidad. OBJETIVO: Describir y analizar PG de niños > 1 año con sospecha de TRS. PACIENTES Y MÉTODO: Se incluyeron PG de niños y adolescentes > 1 año de edad con sospecha de TRS, desde diciembre de 2011 hasta agosto de 2017 provenientes de la ciudad de Concepcion, Chile. Se recopilaron datos demo gráficos, clínicos y variables poligráficas. Estadística descriptiva, expresando resultados en mediana y rango. Se determinó asociación entre índice de apnea hipopnea (IAH) y saturación mediante Rho de Spearman; considerando significancia p < 0,05. RESULTADOS: Se analizaron 190 estudios. Edad 7,9 años (1,0-20,6), varones 61%. Diagnósticos: enfermedades neuromusculares (ENM) (24,2%), daño pulmonar crónico (21,1%), obstrucción de vía aérea superior (OVAS) (19,5%), daño neurológico (11%), síndrome de Down (8,9%) malformaciones VAS (7,4%), hipoventilación central (3,7%), obesidad (2,6%) y otros (1,6%). El 55,3% de las PG resultaron alteradas; síndrome de apnea hipopnea obstructiva del sueño (SAHOS) leve en 53,3%, moderado 30,5% y severo 16,2%. No se observaron diferencias significativas en IAH entre grupos de patologías (p = 0,032), destacando un mayor IAH en Obesidad 9,0 (0,41-51) y ENM 23,9 (0,4-36,6). Se constató asociación entre IAH y parámetros de saturación; saturación promedio (rho = -0,425; p = 0,001); mínima (rho = -0,654; p = 0,001); y saturación bajo 90% (rho = 0,323; p = 0,001) en la totalidad de la muestra. DISCUSIÓN: Existió un alto porcentaje de SAHOS en pacientes pediátricos de riesgo, en especial en aquellos con ENM y obesidad. La PG es una herramienta accesible e implementable en un hospital público; situación potencialmente extrapolable a otros centros asistenciales.

Introducción. Los eventos aparentemente letales (Apparent Life-Threatening Event, ALTE) son causa frecuente de hospitalización en lactantes. Hay poca información sobre el enfoque estandarizado para establecer su etiología, a pesar de que un diagnóstico causal correcto puede afectar la evolución clínica, la duración de la hospitalización y los recursos sanitarios. Objetivo. Comparar los efectos del enfoque diagnóstico estandarizado en lactantes hospitalizados por este tipo de eventos. Materiales y métodos. Se hizo un estudio retrospectivo de cohorte con base en los datos recolectados de las historias clínicas de lactantes hospitalizados por esta causa en el servicio de pediatría entre el 2002 y el 2009. Se analizaron dos cohortes de pacientes agrupados según su manejo: la cohorte 1, con guías clínicas, y la cohorte 2, con guías clínicas, protocolo de estudio y seguimiento ambulatorio. Se compararon los grupos en cuanto a la etiología, el tiempo de hospitalización y la tasa de nuevas hospitalizaciones. Resultados. De los 255 lactantes hospitalizados por eventos aparentemente letales, el 57,6 % integró la cohorte 1 y, el 42,3 %, la cohorte 2. No se observaron diferencias en cuanto a la edad y el sexo. En la cohorte 2 se observó un mayor porcentaje de causas atribuidas (63,9 Vs. 87,0; p<0,0001), y un menor tiempo de hospitalización (8,0 Vs. 5,0 días; p=0,0001). No hubo diferencias en cuanto a nuevas hospitalizaciones (10,5 Vs. 8,3 días; p=0,7435). Conclusiones. El enfoque del manejo de lactantes afectados por eventos aparente letales basado en protocolos, se asoció con un mayor porcentaje de reconocimiento de las causas atribuidas y con un menor período de hospitalización. A partir de estos resultados es posible sugerir la implementación de este tipo de estandarización para el manejo de dichos pacientes.


Introduction: An apparent life-threatening event (ALTE) is a frequent cause of hospitalization in infants. However, there is little evidence about the existence of a standardized approach to discover the main etiology, although a correct causal diagnosis can affect clinical evolution, hospital stay, and health resources. Objective: To determine the effects of a standardized diagnostic approach in infants admitted with ALTE. Materials and methods: We conducted a retrospective cohort study with the data collected from clinical records of infants hospitalized for ALTE in the pediatric unit between 2002 and 2009. Two cohorts of patients were analyzed according to the procedures defined for these cases: Cohort 1 with clinical guidelines and cohort 2 with clinical guidelines, study protocol, and outpatient follow-up. Etiological causes, hospitalization periods and readmission rates were compared between both cohorts. Results: Of the 255 infants hospitalized for ALTE, 57.6% corresponded to cohort 1 and 42.3% to cohort 2. No differences were observed in age and gender between groups. The highest percentage of attributed causes (63.9 vs 87.0%; p<0.0001) and a shorter period of hospitalization (8.0 vs 5.0 days; p=0, 0001) were observed in cohort 2. No differences in hospital readmission were observed (10.5 vs 8.3 days; p=0.7435). Conclusions: The protocol-based approach for infants with EAL was associated with a higher percentage of recognition of attributed causes and a shorter hospitalization period. Therefore, our results allow recommending this type of standardization for the management of these patients.

INTRODUCTION: The 6-minute walk test (SMWT) is a submaximal exercise test widely used in the clinical setting, which allows the assessment of physical capacity in healthy and chronic children. For a proper interpretation of SMWT it is necessary to compare the value obtained by the patient with a normal value appropriate for the studied population. The aim of this study was to analyze the normal values of SMWT, in order to establish grouped normal values and to evaluate their association with anthropometric and racial variables in healthy children and adolescents. METHODS: Systematic review performed in 6 electronic databases. We included studies aimed at determining normal values of SMWT in healthy children and adolescents. Normal values of the SMWT were weighted according to the sample size and their association with the gender, age and continent of origin of the study was evaluated. Additionally, the correlation coefficients between the distance during SMWT and the pre dictor variables were meta-analyzed using the random effects model. RESULTS: The review included 10 articles that included 5352 individuals (51.9% men). The weighted average of the distance dur ing SMWT was 619.8 m, there being a difference between the age, gender and continent of origin groups. The meta-analysis of random effects revealed a moderate association between the distance and height, weight and age variables. CONCLUSION: SMWT is associated with gender, continent and anthropometric variables. However, the predictive variables determine a low percentage of normal walked distance in children and adolescents.

INTRODUCCIÓN: El test de marcha de 6 minutos (TM6') es una prueba de ejercicio submáximo ampliamente utilizada en el contexto clínico, la cual, permite evaluar la capacidad física en niños sanos y con enfermedades asociadas. Para una adecuada interpretación del TM6' es necesario comparar el valor obtenido con un valor normal apropiado para la población de interés. El objetivo de este trabajo fue analizar los valores de normalidad del TM6', establecer valores de normalidad agrupados y evaluar su asociación con variables antropométricas y raciales en niños y adolescentes sanos. MÉTODOS: Revisión sistemática de la literatura realizada en 6 bases de datos electrónicas. Se incluyeron estudios de corte transversal que reportaran valores normales del TM6' en niños y adolescentes sanos. Los valores normales de la distancia recorrida (DR) fueron ponderados de acuerdo al tamaño muestral y se evaluó su asociación con el género, la edad y continente de procedencia del estudio. Los coeficientes de correlación entre la distancia recorrida y las variables predictoras fueron metaanalizados mediante el modelo de efectos aleatorios. RESULTADOS: A la revisión ingresaron 10 artículos que consideraron 5.352 individuos (51,9% hombres). El promedio ponderado de la DR fue de 619,8 m existiendo diferencia entre los grupos de edad, género y continente de procedencia. El metaanálisis de efectos aleatorios reveló una moderada asociación entre la DR y las variables estatura, peso y edad. CONCLUSIÓN: El TM6' se asocia con el género, continente y variables antropométricas. Las variables predictoras determinan un bajo porcentaje de la DR normal en niños y adolescentes.

Introduction: An apparent life-threatening event (ALTE) is a frequent cause of hospitalization in infants. However, there is little evidence about the existence of a standardized approach to discover the main etiology, although a correct causal diagnosis can affect clinical evolution, hospital stay, and health resources. Objective: To determine the effects of a standardized diagnostic approach in infants admitted with ALTE. Materials and methods: We conducted a retrospective cohort study with the data collected from clinical records of infants hospitalized for ALTE in the pediatric unit between 2002 and 2009. Two cohorts of patients were analyzed according to the procedures defined for these cases: Cohort 1 with clinical guidelines and cohort 2 with clinical guidelines, study protocol, and outpatient follow-up. Etiological causes, hospitalization periods and readmission rates were compared between both cohorts. Results: Of the 255 infants hospitalized for ALTE, 57.6% corresponded to cohort 1 and 42.3% to cohort 2. No differences were observed in age and gender between groups. The highest percentage of attributed causes (63.9 vs 87.0%; p<0.0001) and a shorter period of hospitalization (8.0 vs 5.0 days; p=0, 0001) were observed in cohort 2. No differences in hospital readmission were observed (10.5 vs 8.3 days; p=0.7435). Conclusions: The protocol-based approach for infants with EAL was associated with a higher percentage of recognition of attributed causes and a shorter hospitalization period. Therefore, our results allow recommending this type of standardization for the management of these patients.


Introducción. Los eventos aparentemente letales (Apparent Life-Threatening Event, ALTE) son causa frecuente de hospitalización en lactantes. Hay poca información sobre el enfoque estandarizado para establecer su etiología, a pesar de que un diagnóstico causal correcto puede afectar la evolución clínica, la duración de la hospitalización y los recursos sanitarios.Objetivo. Comparar los efectos del enfoque diagnóstico estandarizado en lactantes hospitalizados por este tipo de eventos.Materiales y métodos. Se hizo un estudio retrospectivo de cohorte con base en los datos recolectados de las historias clínicas de lactantes hospitalizados por esta causa en el servicio de pediatría entre el 2002 y el 2009. Se analizaron dos cohortes de pacientes agrupados según su manejo: la cohorte 1, con guías clínicas, y la cohorte 2, con guías clínicas, protocolo de estudio y seguimiento ambulatorio. Se compararon los grupos en cuanto a la etiología, el tiempo de hospitalización y la tasa de nuevas hospitalizaciones.Resultados. De los 255 lactantes hospitalizados por eventos aparentemente letales, el 57,6 % integró la cohorte 1 y, el 42,3 %, la cohorte 2. No se observaron diferencias en cuanto a la edad y el sexo. En la cohorte 2 se observó un mayor porcentaje de causas atribuidas (63,9 Vs. 87,0; p<0,0001), y un menor tiempo de hospitalización (8,0 Vs. 5,0 días; p=0,0001). No hubo diferencias en cuanto a nuevas hospitalizaciones (10,5 Vs. 8,3 días; p=0,7435).Conclusiones. El enfoque del manejo de lactantes afectados por eventos aparente letales basado en protocolos, se asoció con un mayor porcentaje de reconocimiento de las causas atribuidas y con un menor período de hospitalización. A partir de estos resultados es posible sugerir la implementación de este tipo de estandarización para el manejo de dichos pacientes.
